Elbow CPM Following a Stable Intra-Articular Fracture

چکیده

CPM after a stable fracture has been used extensively in the elbow.9,19,20,21,22,28,30,43,46,48 The initial goal of therapy following a stable fracture is to maintain the range-of-motion. If passive motion is not started within the first 48 hours the prognosis for improvement is significantly diminished.10 O’Driscoll and Giori36 have demonstrated that CPM immediately following a surgical procedure acts to pump blood and edema fluid out of the joint and periarticular tissues. The reduction of these fluids from a synovial joint reduces the risk of post-surgical joint stiffness. A contracted joint typically has an inflammatory component which can be aggravated by the surgical release itself resulting in limited or no improvement in range-of-motion following the surgical procedure. Salter,42 Kim,26 Kroeder27 and Moran34 have all shown that CPM has reparative effects on inflamed joints. However, until recently the mechanism by which CPM acts as an anti-inflammatory agent was unknown. Recent studies by Gassner,17 Lee,29 Xu49 and Ferretti14 have helped explain the molecular basis for the beneficial effects of CPM on the inflamed joint. A CPM device by applying cylic tensile stress on the involved joint for an extended time counteracts the effects of the inflammatory agents even better than immobilization.

Corrective Osteotomy for Intra-Articular Distal Humerus Malunion

Background: An intra-articular distal humerus malunion can be disabling. To improve function, reduce pain and/ or prevent further secondary osteoarthritis an intra-articular corrective osteotomy can be considered. Herein we present the indications, practical guidelines for pre- operative planning and surgical technique. Subsequently, we provide long-term results in a small series.   Meth...

Terrible triad of the elbow: a case report of a new variant.

The usual terrible triad of the elbow consists of posterior dislocation of the elbow, radial head fracture and coronoid fracture. We describe a new variant of the terrible triad of the elbow consisting of fracture of the capitellum involving the full length of the trochlea and posterolateral dislocation of the elbow associated with coronoid fracture (type 1 Regan-Morrey). A 25-year-old girl was...

The effect of haematoma aspiration on intra-articular pressure and pain relief following Mason I radial head fractures.

BACKGROUND The aspiration of the accompanying haematoma by Mason type I radial head fractures is advocated by several authors to achieve an analgesic effect. The purpose of this study was to investigate the effect of haematoma aspiration on intra-articular pressure and on pain relief after Mason I radial head fractures. MATERIALS AND METHODS A total of 16 patients (10 men and six women, age 2...

Outcomes of open arthrolysis of the elbow without post-operative passive stretching.

The use of passive stretching of the elbow after arthrolysis is controversial. We report the results of open arthrolysis in 81 patients. Prospectively collected outcome data with a minimum follow-up of one year were analysed. All patients had sustained an intra-articular fracture initially and all procedures were performed by the same surgeon under continuous brachial plexus block anaesthesia a...

Elbow Passive Motion Rehabilitation Utilizing a Continuous Passive Motion Device following Surgical Release, Manipulation Under Anesthesia, or Post Stable Fracture: A Review

Background Post-operative rehabilitation of the elbow following a stable fracture, surgical release or manipulation under anesthesia consists of a period of passive motion (PM) to prevent adhesions, the detrimental effects of immobilization, reduce pain, reduce inflammation and increase range of motion to a functional level. Continuous passive motion (CPM) devices have been used routinely as a ...
